Over time, this can cause the breast to “bottom out,” losing fullness on top while gaining weight and volume below, which is the opposite of an aesthetically pleasing result. The technique also produces a long scar along the inframammary fold, which tends to thicken over time.

Several surgeons worked to address these limitations. Dr. Pfeifer’s approach combines a vertical scar pattern with a superior pedicle. The superior pedicle allows tissue to be removed from the lower half of the breast, which is the area most responsible for sagging. The vertical, or “lollipop,” scar pattern also eliminates the long inframammary and lateral scars that are prone to thickening. Dr. Pfeifer sometimes adds a short scar along the inframammary fold to further refine the shape. Most teenage patients are excellent candidates for this technique, which typically results in a natural, aesthetic shape with minimal scarring.

For patients who need an inferior pedicle breast reduction with a Wise, or anchor pattern scar, typically those with extremely large breasts. Dr. Pfeifer uses this approach only when necessary. It also provides a stronger blood supply to the nipple-areola complex, which is especially important when a very large amount of tissue is being removed.

What Can Be Done to Produce Fine, Imperceptible Scars?

She uses Quill® sutures, developed by plastic surgeon Dr. Gregory Ruff, to further minimise tension on the skin. She also uses the Brijjit® force-modulating tissue bridge developed by plastic surgeon Dr. Felmont Eaves III, which provides additional support to the closure. A specialised taping system is also used to stabilise the skin edges as they heal.

A supportive bra is also recommended after surgery to help reduce pressure and weight on the lower half of the breast while the area heals.

For patients with more pigment in their skin, who may be prone to hyperpigmentation around the perimeter of the scar, Dr. Pfeifer may recommend pretreating the skin with hydroquinone before surgery and resuming the regimen several weeks after the procedure.
