This 49 year old professional woman wanted to improve her result after subpectoral augmentation with shaped saline implants 11 years prior to her visit to my office. She felt the implants were displaced to the side, the implants felt firm and the right implant did not appear to be correctly oriented. On examination her right implant appeared to have turned, the breast felt firm and the implants were very displaced laterally. We discussed her options and she elected to change to shaped silicone implants with pocket correction. I performed a neosubpectoral pocket with Strattice repair of the lateral pocket to get better implant position and more cleavage. The implant removed was a shaped saline filled implant with an oval base, 350cc. The right implant was turned and not correctly oriented. The new implants are Sientra shaped silicone, round base, high profile, 370cc. After the revision the implants are soft, well positioned and the patient is very happy with her result.
