47 year old woman who had augmentation with saline implants 4 years before coming to my office. Allergan style 68, round smooth saline, 270cc filled to 300cc. Prior to deflation the right breast felt firm, consistent with capsular contracture. On exam the left felt firm, consistent with capsular contracture. She also had ptosis of the breast tissue. During her revision surgery the right implant was found in the subpectoral pocket and on the left side it was in the subglandular pocket. She underwent revision with total capsulectomy, removal of the saline implants, subpectoral augmentation with shaped silicone implants, Allergan style 410MF (moderate height, full projection)375cc on the right, 335cc on the left and superior pedicle mastopexy with limited scar. Post-operatively the patient recovered quickly. The right breast is slightly more ptotic than the left but the is happy with the size and shape of her breasts. She is now 10 years post-op and her breasts remain soft.
