This 46 year old mother of two presented for revision of breast augmentation consultation. She had subpectoral breast augmentation with silicone implants, via peri-areolar incisions several years prior. Her goals were to improve the cosmetic result of the augmentation, correct the displacement of the implants as well as the capsular contracture, which made the breasts feel very firm. The left silicone implant was ruptured. I revised her augmentation by creating a neo-subpectoral pocket on the right and performed capsulectomy on the left. The surgery was performed through an inframammary fold incision as peri-areolar incisions may have a higher chance of capsular contracture. There are several technical things that I do during the surgery to further reduce the chance of recurrent capsular contracture. Finally, shaped implants have an extremely low capsular contracture rate. The implants are Sientra shaped round base, high profile, silicone, 370cc. The patient is now 1 1/2 year post-op with no recurrent capsular contracture. The displacement has been corrected. She is very happy with the softness of her breasts and how pretty she looks.
