39 year old woman who presented for revision of augmentation. The patient had tuberous breast and had subglandular augmentation with shaped saline implant plus peri-areolar mastopexy when she was a teenager. Several years later, she had a revision for correction of capsular contracture and to decrease the implant size. When sharing her story with me, the patient was very clear that, even from the very beginning, she did not want large breasts. She wanted a pretty shape. Her concerns at the time of her consultation with me were: the wide space between her breasts, poor breast shape, too much upper pole bulging and poor scars around the areola. I revised her augmentation by removing the old saline implants, opening he capsule medially and laterally and placing shaped silicone implants, Allergan style 410 LM, 140cc and revised the peri-areolar mastopexy scars. After the revision surgery, she has an improved shape of the breast, with a narrower distance between the breasts and a more natural upper pole; the scars are barely perceptible. She is thrilled. Currently the patient is 4 years post-op and doing well. Her implants feel soft and her breast looks completely natural.
